Complete the application on the left and submit it to us. We will review the application and make the payment to the camp.

Amy’s Wish will make every effort to pay for summer camp. Unfortunately we’re  limited by the funding and cannot guarantee that all wishes for camp will be granted.

We will respond back to you within 3 to 5 business days regarding the approval of your application.
